Address Boundary

How do I enroll if I do not have a permanent address?

Your address will be the address where your child sleeps at night.  This address will be used to determine your home attendance boundary school.  On the registration form, there will be specific questions about where you live. The Howard-Suamico School District has McKinney-Vento program staff that can provide you assistance with the enrollment process as well as transportation as needed.  If you have questions regarding the McKinney-Vento program, HSSD staff can be reached at 920-662-7260.

 

I am currently living with someone and cannot provide proof of residency. 

Families who have established stable residency in the home of another adult individual residing in the District can complete the Co-Residency Verification Form.   Both the parents/legal guardians and the co-resident will need to complete and sign this form.  The co-resident is the resident adult with whom the parent(s)/legal guardian(s) and student(s) are living and is the adult that can provide proof of residency for the family.

My family recently moved into a different elementary boundary attendance area and I would like my child to continue attending the elementary school they are currently attending.  What do I need to do?

The parent/legal guardian will need to complete the Alternate Building Request Form.  The completed form should be returned to the District Office at 2706 Lineville Rd, Green Bay, WI 54313; fax to 920-662-9777, or email to registration@hssdschools.org.  Parents/legal guardians will receive communication of decision via US mail.  Please be sure to read the information and guidelines on the form. 

 

Guardianship
 

Example:  My niece is living with me and I am taking care of and providing financial support to her.  Her parent/legal guardian resides outside the boundaries of the Howard-Suamico School District.  How do I enroll her in school?

If the student is residing with you for reasons other than for the sole purpose of attending school in the Howard-Suamico School District, the parents/legal guardians needs to complete a Request for Residency and Educational Guardianship Form.  If parents/legal guardians decide to revoke an authorization that has been placed on this form, a Request to Revoke Form needs to be submitted to the District Registrar Office.

The appointed legal guardian will be asked to complete an Educational Guardianship – Acceptance of Responsibility Form.  The appointed legal guardian will complete the online registration form to enroll the child and provide the registration verifications such as birth certificate, immunization record and proof of address.  If the appointed legal guardian decides to revoke an authorization that has been placed on this form, a Request to Revoke Form needs to be submitted to the District Registrar Office.

Example:  My niece currently lives with me as a result of a voluntary agreement between myself and my niece’s parent/legal guardian.  Both myself and my niece’s parent/legal guardian live within the District boundaries.  How do I enroll her in school?

The student’s parent/legal guardian would need to complete the online registration form.  The student’s home attendance boundary attendance school will be based upon the parent/legal guardian’s address.  Both the parent/legal guardian should complete Individual Permission for Rights to Records and/or Educational Decisions Form (parent/legal guardian would check both options on the form). 

 

This form will need either a witness of signature at the school/district or be notarized prior to providing to the district.  If a parents/legal guardians decides to revoke an authorization that has been placed on this form, a Request to Revoke Form needs to be submitted to the District Registrar Office. 

 

Registration

Does the district have paper copies of the registration form?

We do not have paper copies of the registration form.  Our online registration form is accessible anywhere there is internet access.  Each school and district office has a kiosk available for parents/legal guardians to use.  Assistance is also available if needed.  A registration form needs to be submitted for each child.  Once a successful registration is submitted, you may add additional students on to the registration using the SNAP feature.  This feature will copy previously entered demographics, etc. for the next student. Portions of the registration form are student specific.   

After I complete the online registration form, is there anything else I need to provide to the District?

To complete the online registration, parents/legal guardians needs to provide the following verification documents:

  • Child’s Birth Certificate

  • Child’s Immunization Record

  • Parent/legal guardian of child proof of address

    • Utility bills such as WPS, cable, or water bill

    • Parents may also provide a mortgage statement or lease agreement.  

These documents can be brought to the District Office or any school within the district.  District personnel will process these verifications and notify the Registration Office.

 

I am unable to provide the certified birth certificate required to complete my child’s registration. 

The student’s parents/legal guardians can complete the Inability to Provide Certified Birth Certificate Form.  This form AND the legal document provided can be used in lieu of the student’s birth certificate.

Transportation/Busing

My child rides the bus to and from school and I would like to request my child be picked up and/or drop off at a location different than our home address. 

Parent/legal guardians can complete the Special Request for Transportation Change Form.  Submit the completed forms to Lamers Bus Lines at 1934 Cardinal Ln, Green Bay, WI  54313, Fax to 920-434-5200, or email to:  10groupSB@golamers.com

 

Student Demographic/Report Card Questions

 

I changed my cell phone number and email.  How can I notify my child’s school to update this information in my child’s file?

Parents/legal guardians will have a PowerSchool Parent Portal account set up for them.  Parents/legal guardians can log into their account, select the Year-Round Updates icon on the main student screen, and indicate demographic changes.  The Registration Office will receive the information and update our records.  If an address is changed, parents/legal guardians are asked to provide proof of address to the school or district office.  Parents/legal guardians can also contact the school or registration office to make demographic changes.  

I would like to view my child’s report card and attendance. 

Parents/legal guardians can log into their PowerSchool Parent Portal to view their child’s grades and/or attendance.  If parents/legal Guardians need assistance with logging into their PowerSchool Parent Portal, please contact their child’s school.

 

Sharing of student school information

I would like my child’s step-father to receive my child’s report cards/test results, attend my child’s school conferences, and communicate to my child’s teacher and/or school. 

The parent/legal guardian my provide permission by completing the Individual Permission for Rights to Student Records and/or Rights to Make Educational Decisions Form.   If a parent/legal guardian decides to revoke an authorization that has been placed on this form, a Request to Revoke Form needs to be submitted to the District Registrar Office.
